The term: 'body contouring' gives the impression of the human body as a lump of clay that the 'artistic' plastic surgeon sculpts it into a work of fine art - like Michelangelo's David, perhaps. Very few plastic surgeons are artists in the generally accepted meaning of the word - despite self-indulgent posturing, marketing hype and the occasional art creation. The sad truth is that we are dealing with a pre-existing musculoskeletal structure and skin envelope that we can only alter in well-defined and limited ways. We do not have a free-handed liberty to shape the body into whatever form the patient desires.

Body contouring is therefore a misleading term that includes such procedures as liposuction (foolishly and even more misleadingly rendered 'liposculpture' by those with artistic pretensions); excisional surgery after weight loss; body, leg, breast and arm lifts; abdominoplasty and fat injection.

Liposuction (suction assisted lipectomy, liposculpture, suction lipectomy) can remove unsightly bulges of fat from areas where there is a disproportionate amount of it. It has become the most common aesthetic surgery procedure performed in the United States, and is highly effective in the right circumstances. The surgery itself leaves tiny scars. However, if large amounts of fat require removal and if the skin tone is poor - for example, after massive weight loss - the results are frequently disappointing. The loose skin, now unsupported by fat, becomes looser, taking on ripples at best; and, at worst, hanging in folds.

Body contouring involves dealing with the loose skin by removing it and thereby tightening that which remains. Since long incisions are necessary, the scars are more prominent than with liposuction, but are hidden - as far as possible - in inconspicuous places. For example, in arm reductions (batwing correction, brachioplasty) the scar is placed in the armpit or down the inside of the upper arm; in thigh lifts, the scar is located in the groin crease or down the inside of the thigh; in lower body lifts, the scar is sited within the bikini line, and with breast lifts, the scar is positioned around the nipple-areola complex and on the undersurface of the breast.

People who experience massive weight loss, be it from gastric bypass, banding or dieting alone, experience wildly different patterns of skin redundancy. Like all sensible plastic surgery, the techniques used, their combination and their staging have to be individualized to the patient and his or her physical condition. Silicone breast implants were first introduced in the 1960's. Due to controversy in the early 1990's there was a widespread fear about the possibility of silicone breast implants causing autoimmune diseases in women. Extensive studies since then have given no evidence that silicone breast implants have any relationship to breast cancer, autoimmune disease or any other systemic illnesses in patients. Specifically, patients with breast implants have no higher likeliness of diseases such as rheumatoid arthritis, scleroderma or lupus, in comparison than those without breast implants.

The FDA approved the use of the third generation silicone gel implants in November of 2006 for women 22 years of age and older. These implants are not filled with liquid silicone, but rather a cohesive silicone gel.

The increased safety of the cohesive gel implants led to the FDA's approval of these implants for cosmetic use. Saline implants are FDA approved for breast augmentation in women 18 years of age and older. The FDA regulates the age of approved use higher for silicone gel implants to ensure that a woman is emotionally mature enough and fully understands the risks of these implants.

Liposuction is one of the most commonly performed procedures in the United States today. Liposuction can be a safe and effective procedure for patients who have developed areas of subcutaneous fat that is resistant to diet and exercise. Silicone implants were first popularized in the 1960's. Controversy in the early 1990's led to widespread fear about the possibility of silicone breast implants causing autoimmune diseases in women. Extensive studies since then have showed no evidence that silicone breast implants have any relationship to breast cancer, autoimmune disease or any other systemic illnesses in patients. Specifically, patients with breast implants have no higher incidence of diseases such as rheumatoid arthritis, scleroderma or lupus, in comparison with the general population.

The Food & Drug Administration approved the use of the third generation silicone gel implants in November of 2006 for women 22 years of age and older. These implants are not filled with liquid silicone, but rather a cohesive silicone gel.

The increased safety of the cohesive gel implants led to the FDA's approval of these implants for cosmetic use. Saline implants are FDA approved for breast augmentation in women 18 years of age and older. The FDA set the age of approved use higher for silicone gel implants to ensure that a woman is emotionally mature enough and fully understands the risks of these implants.

There is no perfect breast implant and both the saline and silicone gel implants have advantages and disadvantages. The belly button is preserved and repositioned in its correct location, and liposuction is performed in select areas. The final result is a firm, contoured abdomen with a transverse scar in the bikini-line.

Concerns over the potential for silicone to produce autoimmune disease have now been laid to rest and gel-filled implants are back on the market with the blessing of the FDA.

Women request breast enhancement for the same reasons as most people who seek aesthetic surgery: to improve their self-image and attractiveness to others. The operation, usually carried out under general anesthesia as a day case, is relatively simple. The incisions made for the introduction of the implant vary. The most popular route is via a 4-5cm incision in the fold just under the breast. The implant can also be placed through a 'periareolar' incision - one that follows the lower border of the areola - or through the armpit (transaxillary). Inflatable implants have even been inserted through the belly-button using special instrumentation (TUBA). For reasons I would be happy to discuss, I do not favor this last modality.

Many women feel that their breasts do not match their overall body physique. Small breast size may be due to a lack of normal breast development or to a decrease of breast volume following pregnancy or menopause. This procedure is designed to give the female patient a fuller breast.

Your breast tissue is not altered by a breast enlarging procedure. This operation is performed by placing an implant behind your own breast tissue, simply pushing it forward and enlarging the appearance of the breast. The increase in breast size will be limited by the tightness of your skin, but significant improvement can usually be achieved. The final decision regarding implant size must be made by the surgeon during the operation, but size will be discussed with you before the procedure and every effort will be made to create an optimal breast for you.

It is important that you understand that no person is perfectly symmetrical from one side to the other even before a surgical procedure. Every attempt will be made during surgery to minimize your side to side dissimilarities, but such differences always persist to some degree even after the most successful operation.

There is no evidence that breast surgery alters the possibility of developing breast cancer. Mammography techniques have improved in recent years. In spite of these advances, there is a theoretical risk of compromised detection of early breast cancer following an augmentation mammoplasty. If a cancer were to develop, its detection by mammography could be made more difficult, depending on the location of the cancer within the breast, the position and size of the implant and the type of implant employed. Regardless of your decision to have an augmentation mammoplasty or not, it will still be necessary for you to examine yourself monthly for breast lumps and undergo mammography as suggested by your personal physician.

All women, whether or not they are contemplating breast augmentation, are encouraged to obtain a baseline mammogram at age 35 (or even sooner if you have a family history of breast cancer) and a yearly mammogram after the age of 40.

Normal breast function should not be altered by an augmentation procedure in which the incision is placed within the axilla and the implant positioned behind the pectoral muscle. Although not all women are able to breast feed even without a breast operation, this procedure should not alter the ability to breast feed should this become desirable later. One cannot, however, rule out possible effects on future pregnancy and nursing, particularly when incisions are placed around the areolae (“nipple incision”).
